Outreach Case Manager
About the role
The Outreach Case Manager is a member of the Preventing and Ending Homelessness for Adults Without Children Team. The Outreach Case Manager provides rapid re-housing services to unsheltered or chronically homeless individuals in Central Fairfax County with the goal of moving them from homelessness to housing in 30 days. The Outreach Case manager will also provide prevention services to 5 prevention clients a month.
Duties And Responsibilities
- Assessment of housing barriers, needs, and preferences.
- Development of an action plan for locating housing; and assisting program participants in housing search as needed.
- Outreach and negotiation with owners/landlords.
- Aid in submitting rental applications and understanding leases.
- Assessment of housing for compliance with Emergency Solutions Grant (ESG) requirements for habitability, lead-based paint, and rent reasonableness.
- Aid in obtaining utilities and making moving arrangements.
- Tenant counseling.
- Mediation between the program participant and the owner or person(s) with whom the program participant is living, provided that the mediation is necessary to prevent the program participant from losing permanent housing in which the program participant currently resides.
- Credit counseling and other services necessary to assist program participants with critical skills related to household budgeting, managing money, accessing a free personal credit report, and resolving personal credit problems.
- Document all services identified and provided, referrals made, and transportation provided.
- Enter all rapid rehousing, prevention, and outreach data into HMIS within entry timeframe.
- Travel and meet program participants in the community, including places not meant for habitation such as campsites in the woods, and drop-in centers.
- Coordinate services using a wraparound/team approach. Negotiate, facilitate, and coordinate the creative use of community-based service alternatives and assist staff from other human service agencies/organizations in creative use of community resources.
- Provide community case management services to clients who successfully transition to permanent housing for 6 months to 2 years.
- Assist in managing the FACETS budget for program expenditures.
- Participate in program staff meetings and conferences to share ideas and plans; work cooperatively with staff to meet FACETS goals.
- Participate in and conduct in-service training for staff development.
- Actively support FACETS’ mission-oriented outcome evaluation and outcome management.
- Prepare proposals, reports, and statistics for submission to funding sources with the Program Director's supervision.
- Represent FACETS in the community, in accord with Agency Mission, Vision, Values, and Code of Ethics.
- Determine appropriateness of household for receipt of prevention/diversion funds and recommend amount.
- Conduct weekly home visits to complete reassessment of action plans, progress, and presenting issues in order to identify new goals and modify existing goals as appropriate.
- Provide case management, information and referral to supportive services as needed including treatment for medical, dental, mental health, substance abuse problems as well as social services, housing/shelter needs and employment services.
- Use automated technology including HMIS and hard copy files to maintain, update, and report on case data, goal attainment, and outcomes in a timely manner.
- Address and if necessary, diffuse crisis situations with clients. Assess safety issues and assist clients to access emergency services as needed (e.g., mental health services, child protective services, adult protective services, women’s shelter, homeless shelter).
- Perform outreach response to calls from members of the community including religious institutions, citizens and businesses regarding sightings of homeless individuals in the community.
- Perform other duties as assigned.
Required Skills/Abilities
- Bachelor’s degree in applicable human services field.
- Social work/case management or experience with homeless and/or at-risk populations.
- Must be comfortable meeting program participants in locations not meant for habitation.
- Ability to communicate clearly and concisely, both orally and in writing.
- Proficient with Microsoft Office Suite.
- Candidate must possess a valid driver’s license, reliable transportation, good driving record, and personal car insurance.
- Candidate must be able to drive 15 passenger van.
- Candidate will be required to pass criminal background and Child Protective Services background checks.
- Bi-lingual (English/Spanish)
Physical Requirements
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift 15 pounds at times.
- Must be able to attend occasional meetings at other FACETS office locations within Fairfax County, including 3 annual organization-wide fundraising events each year.
Salary Range
$45,000 - $48,000 annually.
