Our Outreach & Case Management Program empowers clients to become more self-sufficient by helping them prioritize their needs and create sustainable plans for long-term improvement. This program serves as a vital link between individuals in crisis and the services, resources, and community connections they need to thrive. Catholic Charities offers this support free of charge, ensuring that all clients receive the help they need to achieve stability.
Responsibilities
- Develop individualized care plans based on client needs assessments, identifying areas for financial, physical, and emotional support.
- Assist clients in enrolling in services like Medicare, food assistance, disability programs, and more.
- Advocate for clients by connecting them to appropriate resources during times of crisis and providing ongoing personal support.
- Monitor client progress and adjust care plans as needed to ensure positive, sustainable outcomes.
- Maintain accurate case records, logging significant events and tracking client progress.
- Collaborate with internal programs and external community partners to provide comprehensive care and ensure clients receive all necessary services.
Requirements
- Bachelor’s degree in social work or a related field (strongly preferred)
- Two years of experience in case management, family services, or social services is ideal
- Travel required
- Valid driver’s license
