Outreach and Enrollment Specialist
Job Summary
The incumbent is involved in a high level of community outreach & engagement, education and enrollment. Meet with families to explain the role of the Health Home and how it can assist the members and the families. Conduct intake interviews with members and families, including enrolling and/or referring members to other community services. Meet in with families in their homes, at clinics, in schools, in the emergency room and at community events.

Education and Credentials
- HS - High School Diploma or GED required.
- NYS Drivers License required upon hire.
- CANS NY 0-5, CANS NY 6-21 preferred.
Experience
- 2 years of experience in direct sales, health/social services experience and/or education preferred.
