Outreach and Engagement Specialist

Principal Duties/Responsibilities

• Completes outreach activities with individuals potentially disengaged from treatment services.
• Affords assistance in reconnecting these individuals with their treatment providers and objectives.
• Engages with clients, families, and other involved parties through phone interactions and/or home visits.
• Builds trust, encourages hope, and ensures ongoing engagement in behavioral health services with the client and family.
• Communicates clinically relevant information with the client's managing clinician.
• Adheres to clinic and AHCCCS-established guidelines regarding outreach/re-engagement expectations.
• Completes necessary documentation promptly to support allowable billing of outreach activities.
• Maintains client records in compliance with Mohave Mental Health Clinic, Inc., ADHS, RBHA/health plans, AHCCCS, and Joint Commission standards.
• Provides customer service to internal and external stakeholders, as evidenced by direct supervision, complaint resolution, and interviews with customers.
• Participates in Performance Improvement processes within the Clinic.

Qualifications and Experience Required

• High school diploma or equivalency or higher.
• Valid AZ driver's license and proof of insurance.
• AZ fingerprint clearance.
• Age 21 or older.
• Certified in CPR and First Aid.
