Responsibilities
- Deliver outpatient primary care to senior patients
- Focus on building relationships with patients through small panel sizes
- Lead and collaborate with a fully supportive care team to provide outstanding care
- Address both medical and social needs of patients
- Care for those with comorbid conditions, the elderly, and/or medically underserved populations
Requirements
- M.D. or D.O. graduate
- Board certified/eligible in Internal Medicine or Family Medicine (required)
- Fellowship training in Geriatrics welcome, but not required
- Active, non-probationary state medical license
- Highly collaborative with a patient-centric orientation
- Confident in delivering high-quality primary care
- Driven to care for patients with comorbid conditions, the elderly, and/or medically underserved
- Passionate about work and energized by being part of a fast-growing organization
