Outpatient Occupational Therapist- Senior Living

BAYADA Home Health Care is seeking an experienced Occupational Therapist for a full-time opportunity performing outpatient home health visits. This role services adult and geriatric patients on a per-visit basis in homes throughout the Sinking Spring area. As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing to clients, enabling them to receive the medical care required to remain at home.

Responsibilities

• Operate at the top of your licensure using current research-based approaches to maximize functional independence for clients.
• Collaborate with home health therapy partners to transition care from home health to outpatient services, ensuring client-centered goals are met in their living environment.
• Educate clients, families, and community caregivers on care plans to foster a team approach for achieving functional independence.
• Perform outpatient therapy evaluations to develop individualized plans of care for patients in senior living communities or group homes for adults with intellectual disabilities.
• Conduct evaluations, treatments, and discharge planning using an outpatient EMR platform to document treatment approaches tailored to client needs.

Requirements

• Current Pennsylvania Occupational Therapist license.
• Minimum of one year of recent work experience as an Occupational Therapist (prior home care experience preferred but not required).
• Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
