Outpatient Care Coordinator - Social Work
About the role
The Social Worker at St. Tammany Health System intervenes with patients who have complex psychosocial needs, assists with eligibility for social programs and funding sources, coordinates a plan of care for high-risk patient populations, and offers crisis intervention to patients and families.
Responsibilities
- Intervene with patients who have complex psychosocial needs
- Aid in eligibility determination for social programs and funding sources
- Coordinate a plan of care for high-risk patient populations
- Offer crisis intervention to patients and families with psychosocial needs
- Receive referrals from interdisciplinary team members
Requirements
- LMSW or higher licensure required, LCSW preferred
- Three (3) or more years hospital social work or protective services experience preferred
- Experience in psychosocial and therapeutic counseling preferred
- Working knowledge/experience in case management, payors and reimbursement
- Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity
- Basic understanding of Medicare, Medicaid, and Managed Care regulations
- Organized and efficient in establishing priorities and performing tasks within specific timeframes
- Self-motivated requires minimal supervision
- Views problems as a challenge and investigates alternatives which demand creativity, flexibility, and the ability to work under stress
- Maintains strict confidentiality of all verbal & written information and documents
- Familiarity with and appropriate use of community resources
Physical Demands
- Must possess good physical health
- Standing, sitting or walking for long periods of time
- Lifting at least 10 pounds required
Pay
Compensation is commensurate with experience.

Scheduled Weekly Hours: 40 Work Hours Monday - Friday 8:00 AM - 4:30 PM
