Outpatient Care Coordinator

New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast (and beyond), serving 15,000 children and adults annually through inpatient crisis services, a 24-hour help line and mobile response team, and outpatient programs conveniently located across Indian River, Martin, Okeechobee, and St. Lucie counties. We assist 22,000 students in area schools and work closely with courts, law enforcement, jails, and hospitals to improve individual health and community quality of life.

About the role

We are seeking an Outpatient Care Coordinator to support our psychiatric outpatient team, with a primary focus on medication coordination. This role manages refill requests, resolves prescription issues, and ensures timely follow-up with providers and pharmacies. The position works in coordination with front office staff to support scheduling, patient communication, and overall clinic flow while ensuring documentation is accurate and completed on time.

Responsibilities

  • Welcome and assist clients in a professional and friendly manner
  • When appropriately credentialed, perform vital signs as directed
  • Assist clients with telehealth appointments
  • Ensure all medication-related calls are returned within one business day
  • Manage refill requests, resolve prescription issues, and coordinate with providers and pharmacies without making clinical decisions
  • Coordinate client services in collaboration with outpatient psychiatric providers and staff
  • Communicate with clients regarding appointments, medications, and follow-up needs
  • Work with scheduling staff to arrange appointments
  • Enter and maintain accurate information in the EHR
  • Complete required forms and documentation, both paper and electronic
  • Complete FASAMS admissions and quarterly updates
  • Ensure documentation is accurate, complete, and timely
  • Perform other duties as assigned

Requirements

  • High school diploma or equivalent required
  • 1–3 years of experience in a healthcare, behavioral health, or outpatient setting
  • Proficiency in Microsoft Office (Outlook, Word, Excel)
  • Must pass Level 2 Background through DCF and AHCA Clearinghouse

Qualifications

  • Preferred: Experience with scheduling, referrals, insurance authorizations, or care coordination
  • Bilingual skills are a plus
  • Associate’s or Bachelor’s degree preferred
