About the role
Looking for a career where your work truly matters? Heritage Behavioral Health Center is hiring a Care Coordinator to join our outpatient mental health and substance use teams. We're a Certified Community Behavioral Health Clinic serving individuals across a multi-county area in and around Decatur, Illinois.
Responsibilities
- Ensure clients get connected to behavioral health treatment, primary care, specialty care, and community resources
- Follow up proactively with clients to track progress and troubleshoot barriers to care
- Cook up appointments and treatment plans with primary care teams, behavioral health providers, and community partners
- Educate clients and families on care continuity and available resources
- Document client progress and service engagement in electronic health records
- Help clients navigate healthcare and social service systems
- Participate in interdisciplinary team meetings and ongoing trainings
Requirements
- A High School Diploma/GED plus 5 years of supervised mental health or social service experience, OR a Bachelor's degree in social work, psychology, education, or a related human services field
- Minimum age of 21
- Valid driver's license, reliable transportation, and proof of auto insurance
- Strong communication skills and the ability to build rapport with diverse clients
- Comfort with electronic health records (EHR) and Microsoft Office
- A collaborative, adaptable, and empathetic approach to client care

Pay
$23.00–$25.00/hour, based on education, experience, and licensure/certification
Schedule
Monday–Friday, 8 AM–5 PM. Every other Friday off (paid wellness day)
