About the role

Habitat Health is seeking enthusiastic and dedicated professionals to join our growing team. We are expanding our PACE model to meet the needs of aging populations, providing comprehensive medical care and support services to older adults.

Responsibilities

• Provide individualized medical care and support services to participants in our centers and homes.
• Collaborate with interdisciplinary teams to develop and implement care plans tailored to each participant's needs.
• Assist with daily activities and provide in-home support as needed.
• Conduct assessments and evaluations to monitor participant health and progress.
• Participate in regular training and professional development opportunities to enhance skills and knowledge.

Requirements

• Valid driver’s license and reliable transportation.
• Current CPR certification.
• Experience in healthcare or related field preferred.
• Strong interpersonal and communication skills.
• Ability to work flexible hours, including evenings and weekends.
• Pass background check and drug screening.

Qualifications

• Bachelor's degree in a healthcare-related field or equivalent experience.
• Current licensure/certification in relevant fields (e.g., RN, PT).
• Knowledge of PACE program operations and best practices.

Skills

• Strong clinical judgment and problem-solving abilities.
• Effective communication and relationship-building skills.
• Proficiency in Microsoft Office Suite.
• Ability to manage multiple tasks and prioritize responsibilities.
