Orthopedic At-Home Physical Therapist - Per Diem (Staten Island)

About the role

JAG Physical Therapy is seeking an experienced Orthopedic At-Home Physical Therapist to provide care to patients recovering from total hip and knee replacement surgeries and other orthopedic conditions. The ideal candidate will have a strong background in orthopedic rehabilitation and a passion for helping patients regain mobility and independence in the comfort of their own homes.

Responsibilities

  • Evaluate and treat patients for the duration of their plan of care in their home setting
  • Transfer, ambulate, and perform stair negotiation with patients to ensure they are safe for discharge to home
  • Collect and record patient care data following department policies and standards
  • Prepare for evaluations, obtain patient histories, and administer tests and measures
  • Formulate treatment plans based on evaluation results and best practices
  • Collaborate with interdisciplinary teams and patients’ support systems
  • Provide education and training to patients and their support systems
  • Prescribe and train on rehabilitation equipment
  • Communicate with Manager/Patient Access team for scheduling evaluations
  • Attend annual JAG Billing and Coding seminar
  • Provide data and note writing using Prompt EMR for all treatments provided

Requirements

  • Bachelor’s or Master’s Degree in Physical Therapy, Doctorate degree preferred
  • Licensed Physical Therapist in the state practicing in
  • Completed state regulated CEU requirement in appropriate state (NY, NJ)
  • Valid CPR License
  • Experience/knowledge with an EMR software is preferred
  • Excellent oral, written communication skills
  • Strong clinical decision-making skills
  • Excellent work ethic and dedication to patient success
  • Basic computer skills
  • Maintains a positive, professional demeanor at all times
