Operations Case Manager

About the role

The Case Manager provides comprehensive case management tracking and follows up with the patient regarding their services. This includes intake assessments, benefit assessments, and booking referrals. The Case Manager also conducts research and reviews patient files to identify social or medical deterrents to achieving good health. Independently manages a patient caseload.

Responsibilities

• Works directly with clinical staff and outside facilities to provide appropriate case management to include callback lists, postcards, and follow-up on referral statuses and outcomes.
• Communicates referral information to patients, referral agencies, and OneWorld providers as appropriate.
• Provides the patient with written instructions on how to navigate their appointment (e.g., appointment time, location, floor, procedure, pre-op instructions, interpreter services, etc.).
• Issues written discharge instructions to patients and confirms follow-up appointments.
• Serves as a subject matter expert with agency partners (Hope, Every Women Matters, etc.), ensuring program compliance.
• Updates the treatment team on changes as necessary.
