About the role
Provides individualized, age-specific nursing care from admission through discharge, collaborating with the healthcare team to develop and implement evidence-based care plans, document patient progress, and ensure continuity of care.
Responsibilities
- Assess and record patient status, identify care needs, and organize assessment data accurately and completely.
- Validate nursing diagnoses with the patient/family and healthcare team; document diagnoses to support expected outcomes and care planning.
- Initiate and update a Problem List at admission and as needed; incorporate age-specific care principles into the patient’s plan.
- Discuss and agree on the individualized plan of care with the patient/family; begin discharge planning at admission and involve the patient/family and care team.
- Identify and address patient teaching needs throughout the hospital stay; arrange conferences with the patient/family and care team when indicated.
- Prioritize and deliver evidence-based nursing interventions; document assessments, interventions, and patient responses.
- Review patient health data regularly to update the Plan of Care; revise goals systematically based on measurable outcomes.
- Communicate patient progress and care needs to the healthcare team; present relevant information at multidisciplinary meetings.
- Make appropriate referrals to ensure continuity of care and reassess nursing actions for effectiveness.
Qualifications
- Bachelor of Science in Nursing (BSN) required; Associate Degree in Nursing (ADN) accepted with commitment to obtain BSN within three years of first clinical role.
- Current State of Illinois Registered Professional Nurse licensure.
- American Heart Association Basic Life Support (BLS) certification.
- Orthopedic experience required.
- Strong interpersonal, collaborative, and analytic abilities.
- ACLS, PALS/ENPC, and TNCC within one year of hire for Emergency Room roles; TNCC within one year for Pediatric Emergency Room roles.

Pay
Hourly compensation ranges from $34.50 to $55.20.
