Oncology Transitions of Care Nurse (11:30AM - 8:00PM EST)

About the role

This role serves our members diagnosed with cancer. It reports to the RN Care Team Lead and involves conducting clinical assessments, monitoring health changes, coordinating care, educating members and caregivers, and assisting with administrative projects.

Responsibilities

  • Conduct clinical assessments and monitor for changes in health
  • Coordinate care, including transitions, and educate members and caregivers about their diagnosis and treatment
  • Help improve Thyme Care’s service offerings by communicating member and provider feedback to our clinical leadership
  • Assist with other administrative projects as needed
  • Be available for urgent clinical escalations and clinical consult support

Requirements

  • An active, unrestricted compact Registered Nurse (RN) license
  • At least 5 years of nursing experience with 3 years of oncology nursing or case management experience
  • A degree in Nursing (ADN or BSN), with BSN preferred
  • A commitment to obtain OCN certification within 2 years of hire at Thyme Care
  • Fluency in Spanish, both written and verbal, is preferred

Qualifications

  • Strong clinical focus, supporting the need for culturally competent care
  • Comfortable engaging and supporting members, reflecting in efficiency metrics and quality standards
  • Ability to identify and prioritize a member's needs and help them remain safe in the community
  • Experience with video chatting, Google Suite, Slack, electronic health records, or comfort in learning new technology
  • Experience with conducting telephonic assessments, including pain assessments and medication reconciliation
  • Experience with virtual home safety evaluations and assessing the need for DME/supplies
  • Experience providing referrals to PT, OT, skilled nursing, palliative care, hospice care, etc., as appropriate

  • Compensation for up to 2 holidays per year, at a 1x hourly rate and a 2.5x holiday rate
