About the role
This role serves as an Oncology Nurse - Transitions of Care for Thyme Care Medical PLLC, a medical practice. Reporting to the RN Care Team Lead, you will conduct clinical assessments, monitor health changes, coordinate care, and educate members and caregivers.
Responsibilities
- Conduct clinical assessments and monitor for health changes
- Coordinate care, including transitions, and educate members and caregivers
- Help improve Thyme Care’s service offerings by communicating member and provider feedback
- Assist with other administrative projects as needed
- Conduct telephonic assessments, including pain assessments and medication reconciliation
- Ensure members have access to medications and appointments, providing referrals and support
- Perform virtual home safety evaluations and assess the need for DME/supplies
- Provide referrals to PT, OT, skilled nursing, palliative care, hospice care, etc.
- Partner with non-clinical Care Team members to support the member’s social determinants of health needs
- Be available for urgent clinical escalations and clinical consult support
Qualifications
- An active, unrestricted compact Registered Nurse (RN) license
- A degree in Nursing (ADN or BSN), with a BSN preferred
- At least 5 years of nursing experience with 3 years of oncology nursing or case management experience and a Certified Case Manager (CCM)

- Experience with video chatting, Google Suite, Slack, electronic health records, or comfort in learning new technology

- Compensation of $46.35/hour
- Compensation for up to 2 holidays per year, compensated at a 1x hourly rate and a 2.5x holiday rate

Schedule
Shifts are 8:30 am-5:00 pm EST or 11:30 am-8 pm EST, with the possibility of working up to 2 holidays per year.
Location
You must be located within the lower 48 United States to perform this role.
Additional Information
- OCN certification is required at time of hire or a commitment to obtain within 2 years of hire at Thyme Care
- Fluency in Spanish, both written and verbal, is preferred
