About the Role

The Clinical Staff Pharmacist will provide excellent customer service to patients and other members of the interdisciplinary team and serve as a clinical leader to students, residents, and other staff members. This role involves active participation in department programs and clinical/professional development projects, precepting and mentoring pharmacy students on IPPE and APPE rotations, and serving as the primary preceptor to PGY-1 residents. The pharmacist will assist the Residency Program Director, serve on the Residency Advisory Committee, round on patients, develop relationships with the interdisciplinary healthcare team, provide medication therapy plans, monitor patients, and contribute to the creation and implementation of clinical services.

Responsibilities

  • Supervise non-pharmacist personnel by assigning duties, checking all orders, maintaining efficient workflow, and verifying IVs and other medications prepared by technicians.
  • Assess the need for non-formulary items according to department policy and initiate contact with the prescribing physician for substitutes.
  • Verify physicians’ orders for proper dose, regimen, allergies, potential drug-drug interactions/incompatibilities, adverse drug reactions, and monitor lab data before dispensing medication. Utilize appropriate communication logs to notify physicians/nurses of potential problems.
  • Verify all narcotic orders, maintain perpetual inventory logs, perform weekly C-II inventory, and conduct monthly controlled drug inventory. Ensure all forms are completed and filed appropriately.
  • Advise other healthcare professionals on drug interactions, incompatibilities, side effects, and proper administration of medications.
  • Actively participate in department programs and projects.

Requirements

  • BS degree in Pharmacy; Pharm D degree preferred.
  • Hospital experience and/or residency preferred.
  • CT Pharmacist license.
  • Competency in providing direct patient care to neonate, pediatric, adolescent, young adult, adult, and geriatric patients.
