Oncology Nurse Navigator - Oncology Service Line

Responsibilities

• Utilizes the nursing process in providing nursing care to patients in all settings in collaboration with other health care providers.
• Accomplishes professional development and practice within the scope of care defined by law.
• Delegates care and participates in performance and practice improvement activities.
• Delivers nursing care reflecting the mission, vision, values, and philosophies of Kettering Health Network and nursing departments.
• Possesses oncology-specific clinical knowledge and offers individual assistance to patients, families, and caregivers to help overcome health care system barriers.
• Uses the nursing process to provide education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum.
• Acts as a patient advocate and guide to help remove barriers to care.
• Directs patients and families to available resources and supportive services; provides education or referrals for coping with the diagnosis.
• Facilitates a team approach, composed of the patient, family, physician, nurse, social service, and other direct and indirect providers.
• Interacts with the public, physicians, and staff.
• Serves as Cancer Conference Coordinator by collecting and communicating tumor board cases for presentation, acting as conference facilitator, tracking and uploading conference data for Commission on Cancer reporting, and maintaining conference recommendations.
• Documents and tracks information using a database or electronic medical record.
• Tracks, documents, and reports both externally and internally relevant Navigation activities for stakeholders.
• Pursues continuing education opportunities related to specified program and navigation.

Requirements

• Bachelor of Science in nursing preferred.
• Required Licenses/Certification: Licensed in the state of Ohio as a Registered Nurse, BLS certification, OCN certification preferred.
• Minimum Work Experience: Minimum five (5) years clinical experience required. Recent oncology experience preferred.
