On-Call Social Workers (Per Diem) (Brooklyn/Queens/Nassau and Manhattan)

About the Role

Calvary Hospital and Calvary @Home, located in the Bronx, New York, has been dedicated for over 125 years to the palliative care of advanced cancer and other terminally ill patients and their families. The Hospice Social Worker provides medical social services, psychosocial, and bereavement counseling to hospice patients and families.

Responsibilities

• Provide medical social services to hospice patients and families.
• Deliver psychosocial and bereavement counseling.
• Ensure continuity of care by commuting to multiple territories.

Requirements

• Minimum one (1) year of bereavement counseling experience (preferred).
• At least one year of Hospice or Home Care experience (preferred).
• Excellent interpersonal and communication skills.
• Ability to commute to multiple territories (Brooklyn, Queens, Nassau, and Manhattan).
• Valid driver’s license and a vehicle (not required for positions assigned exclusively to Manhattan territory).
• Demonstrates an ability and attitude aligned with the spirit of caring and tradition at Calvary Hospital.

Qualifications

• MSW from an accredited school of social work.
• One year of related experience.
• New York LMSW or LCSW eligible.

Schedule

Weekdays (5:00 PM – 12:00 AM) and Weekends (8:00 AM – 12:00 AM).

Pay

$95.00 – $125.00 per visit.
