Responsibilities

• Be the Lifeline: Respond to a variety of patient needs, from urgent to routine, providing unwavering support in critical moments.
• Provide Empathetic and Heartfelt Assessments: Conduct comprehensive assessments of patients' physical, emotional, social, and spiritual needs to truly connect with them.
• Maintain Accurate Documentation: Ensure every detail is crucial in delivering high-quality care, both in the patient's home and within our agency and regulatory standards.
• Provide Support for Families: Impact patients' families by sharing your knowledge and expertise, guiding and supporting them in caring for their terminally ill loved ones, offering confidence and reassurance during challenging times.

Requirements

• Bachelor's degree in nursing or healthcare preferred.
• Current unencumbered license as a Registered Nurse (RN).
• Minimum one (1) year of experience as an RN in a medical-surgical/acute care setting.
• Hospice experience is a plus.
• Computer proficiency in typing and various programs, including experience with EMR (Electronic Medical Records).
• Current CPR certification if required by state.

Pay

$68,000.00 - $78,000.00 Salary (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position.
