On Call Registered Nurse

Role: Registered Nurse On Call for hospice care. Responsibilities Provide after-hours hospice care in home and facility settings.Triages calls, conducts urgent visits, and manages symptoms and pain.Implement and monitor hospice care plans.Perform patient assessments and administer medications.Collaborate with physicians and interdisciplinary team members.Ensure accurate documentation of care provided.Support families through education and emotional guidance during end-of-life care. Qualifications & Skills Current Registered Nurse licensure.Strong clinical judgment and autonomous decision-making skills.Excellent communication and documentation abilities.Ability to respond reliably during evenings, nights, weekends, and holidays.
