Role: Registered Nurse On Call Responsibilities Provide after-hours hospice care for patients with life-limiting illnesses in homes and facilities.Triages calls, makes urgent visits, and manages symptoms and pain.Perform patient assessments, administer medications, and implement individualized care plans.Collaborate with physicians and interdisciplinary team members.Educate families and caregivers, ensuring understanding of care and comfort measures.Maintain accurate documentation of patient care and interventions.Support patients and families during crises, promoting dignity and comfort.
