Responsibilities

• Coordinate and supervise on-call care team activities in a fast-paced environment.
• Ensure the delivery of the right care, at the right time, in the right place for patients and their families.
• Support healthcare providers to maximize their impact and efficiency.

Requirements

• High School Diploma or GED.
• 2 years of experience in a healthcare or social service environment; supervisory experience preferred.
• Excellent communication skills.
• Passion for patient care.
• Reliable transportation to perform job responsibilities.

You will report to the Coordination Team Lead. This is a per diem, remote position.
