About the role
We are seeking an on-call Case Manager to join our interdisciplinary clinical team at Priest Drive Recovery Health Center. The ideal candidate will work with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment.
Responsibilities
- Affords clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals
- Completes assessments, crisis plans and individualized service plans
- Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with members, their family, behavioral health, medical and dental providers, community resources and others
- To ensure all services prescribed in the individualized service plan are implemented, conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges
- Performs all case management functions associated with caseload including completing the assessment and service planning processes; identifies the need for further or specialty evaluations
- Pursues best practice outcomes for persons with mental illness including continuing education, employment, independent housing and community tenure
Requirements
No experience required with a bachelor's degree in behavioral health - we will train you. New grads welcome.
Qualifications
Bachelor's degree in Behavioral Health or related field
Skills
Excellent communication skills, ability to work independently and collaboratively, strong organizational skills, and proficiency in Spanish is a plus.
