Duties
- Affords clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals.
- Completes assessments, crisis plans and individualized service plans.
- Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health, medical and dental providers, community resources and others.
- To ensure that all services prescribed in the individualized service plan are implemented, conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges.
- Pursues best practice outcomes for person with mental illness including continuing education, employment, independent housing and community tenure.
Requirements
- No experience required with a bachelors degree in behavioral health - we will train you.
- New grads welcome.

- Full-Time Employed: 40 hours/week M-F 8:30a - 5:30p
