Key Responsibilities
- Conduct patient evaluations and develop individualized OT treatment plans
- Provide skilled occupational therapy interventions and patient education in the home setting
- Monitor and document patient progress and outcomes in the EMR
- Communicate effectively with patients, families, and interdisciplinary teams
- Ensure compliance with home health regulations, physician orders, and documentation standards
- Participate in quality improvement initiatives and care coordination meetings
- Collaborate with clinical, administrative, and operations teams to support patient care and service delivery
- Perform other clinical or administrative duties as assigned
Position Qualifications
- Master’s or Doctorate in Occupational Therapy required
- Current state licensure as an Occupational Therapist
- 1–3 years clinical experience in home health, outpatient, or rehabilitation settings preferred
- Strong assessment, treatment planning, and patient education skills
- Knowledge of home health regulations and documentation requirements
- Proficiency with EMR systems and Microsoft Office Suite
- Excellent written and verbal communication and interpersonal skills
- Ability to work independently and manage a patient caseload
- Availability during business hours with occasional after-hours/weekend coverage
