Staten Island, NY – Personal Touch Home Care is expanding our Special Needs Program and seeking compassionate Occupational Therapists who are passionate about delivering high-quality care in the home setting. This is a fee-for-service, 1099 contracted position. Candidates must have a valid and active NY state license.

Job Overview
As an Occupational Therapist with Personal Touch Home Care, you will assess patients’ needs, develop and implement treatment plans, and help clients improve their daily living and work skills. You will provide rehabilitative services to patients who are unable to travel to a clinic or hospital, focusing on improving quality of life by promoting mobility, function, and independence within the comfort of their own homes.
Responsibilities
- Assess referred patients who require rehabilitation services
- Provide direct patient care to evaluate medical condition, functional capabilities, limitations, and restrictions
- Establish and administer a treatment program with specific goals determined by the patient’s capacity and tolerance under physician direction
- Set realistic and achievable goals for patients
- Document and record the patients’ condition
- Educate patients and families in appropriate therapy methods
- Complete evaluations, weekly progress notes, recertification, and discharge documentation
- Collaborate with all disciplines to provide the best patient-centered treatment and care
- Perform all other duties assigned by the Director of Rehabilitation
- Comply with confidentiality, HIPAA, and Federal Residents Rights
Qualifications
- Graduated from an accredited Occupational Therapist college or university
- Valid professional license and registration for the state of practice (NY)
- Experience with long-term care and/or geriatric care is preferred but not required
- Demonstrates good verbal and written communication and organization skills
- Two years of appropriate experience as an occupational therapist preferred
- Home health experience is preferred
