About the Role
As an Occupational Therapist with Personal Touch Home Care, you will evaluate patients in their homes, develop individualized treatment plans, and provide skilled therapy services that promote independence, mobility, and improved daily functioning. You'll work closely with physicians, patients, families, and interdisciplinary team members to deliver exceptional patient-centered care.
Responsibilities
- Responsible for the assessment of referred patients that require rehabilitation services
- Provide direct patient care to assess their medical condition, functional capabilities, limitations, and restrictions, and potential for rehabilitation
- Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician
- Set realistic and achievable goals for patients
- Document and record the patients’ condition
- Educate patients and families in appropriate therapy methods
- Complete evaluations, weekly progress notes, recertification, and discharge documentation
- Collaborate with all disciplines to provide the best patient-centered treatment and care
- Perform all other duties assigned by Director of Rehabilitation
- Compliance with confidentiality, HIPAA, and Federal Residents Rights
Requirements
- Graduate of an accredited Occupational Therapy program
- Current, active New York State Occupational Therapist license required
- Home health experience preferred
- Experience with pediatric, adult, geriatric, or long-term care populations is a plus
- Excellent communication, documentation, and organizational skills
- Minimum of two (2) years of Occupational Therapy experience (preferred)
Pay
$75.00 per visit to $100.00 per visit
This is a fee-for-service, 1099 contracted position.
