About the Role
As an Occupational Therapist in Home Health, you will provide patient-centered care in home settings, focusing on functional status, activities of daily living, and evidence-based interventions.
Responsibilities
- Conduct comprehensive patient assessments to evaluate functional status and occupational therapy needs.
- Develop and implement individualized care plans in coordination with physicians.
- Perform home assessments and recommend adaptive equipment as needed.
- Observe and report patient responses and changes in condition.
- Provide services within the legal scope of occupational therapy practice using evidence-based techniques.
Requirements
- Current state licensure in Occupational Therapy.
- CPR certification.
- Valid driver’s license and access to reliable transportation.
- Ability to work effectively with diverse patients regardless of background.
