Human Touch Home Health is seeking Occupational Therapists (OTs) for Full-Time, Part-Time, and PRN home health opportunities in Rockville, Gaithersburg, Olney, and surrounding Montgomery County communities. You can focus on Rockville (20850), Gaithersburg and surrounding communities, Olney (all ZIP codes), or a combination of nearby areas, choosing the territory that makes sense for you.

Responsibilities

• Perform comprehensive OT evaluations
• Develop individualized plans of care
• Help patients improve independence with ADLs
• Provide therapeutic interventions to improve functional ability
• Address bathing, dressing, grooming, toileting, meal preparation, and household activities
• Assess home safety and fall risks
• Recommend adaptive equipment and home modifications
• Educate patients and caregivers
• Monitor progress and update treatment plans
• Complete reassessments and discharge planning
• Coordinate with physicians and the interdisciplinary team
• Complete accurate documentation using WellSky

Requirements

• Active Maryland Occupational Therapist License
• Graduate of an accredited Occupational Therapy program
• Reliable transportation
• Strong assessment and treatment skills
• Excellent communication and documentation abilities
• Ability to work independently
• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus
