Occupational Therapist (OT) - Post-Acute Rehab - Skilled Nursing (SNF) | PRN

This PRN opportunity offers flexible scheduling in a skilled nursing and post-acute rehabilitation setting. Whether you're looking to supplement your current caseload or prefer the flexibility of PRN work, we'd love to speak with you.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide evidence-based occupational therapy services.
• Supervise Occupational Therapy Assistants (COTAs) as appropriate.
• Collaborate with physicians, nursing staff, rehabilitation professionals, patients, and families.
• Participate in interdisciplinary care planning.
• Educate patients, caregivers, and facility staff.
• Complete timely documentation and billing.
• Recommend adaptive equipment and discharge planning needs.
• Maintain compliance with state licensure requirements and company policies.
• Demonstrate Concept Rehab's values of Commitment, Respect, and Integrity.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Occupational Therapist (OT) license in the state of practice (or eligible for licensure).
• Excellent communication and teamwork skills.
• Passion for helping older adults maximize function and independence.
