Occupational Therapist (OT), Per Diem

About the Role

We are seeking a compassionate Home Health Occupational Therapist to provide in-home clinical care, promoting patient independence and well-being. This is a Per Diem position serving multiple counties.

Responsibilities

• Conduct home visits for thorough evaluations of patients' functional, cognitive, psychological, and motor abilities.
• Develop and revise individualized intervention plans to improve client outcomes.
• Assess home environments for hazards, recommend equipment and environmental modifications.
• Instruct patients, families, and caregivers on rehabilitative activities and safety protocols.
• Accurately document patient assessments, interventions, and care plans using electronic medical records.
• Collaborate with the interdisciplinary care team to optimize patient care.

Requirements

• Current Missouri Occupational Therapist license.
• Minimum of one year of recent occupational therapy experience.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
• Willingness to travel locally to patient cases.
