Occupational Therapist-OT Per-Diem

Linwood Meadows Care Center is seeking a compassionate and motivated Per Diem Occupational Therapist (OT) to join their outstanding rehab team. Located in Visalia, CA, this position offers competitive pay and a positive, collaborative team environment.

Responsibilities

• Evaluate residents and determine occupational therapy needs and rehabilitation potential
• Develop, implement, and modify individualized treatment plans
• Provide therapeutic interventions focused on ADLs, strength, coordination, balance, cognition, and safety
• Communicate with physicians, nursing staff, residents, and families regarding progress and goals
• Participate in care plan meetings and interdisciplinary team conferences
• Complete timely, accurate, and compliant documentation
• Educate residents and caregivers on home programs, adaptive strategies, and fall prevention
• Recommend adaptive equipment and assistive devices to promote independence
• Supervise COTAs and therapy aides as appropriate
• Maintain a safe treatment environment and follow infection control procedures

Qualifications

• Education: Bachelor’s Degree in Occupational Therapy required, Master’s or Doctorate preferred
• License: Current California Occupational Therapy license in good standing
• Preferred Skills: Experience with Casamba and PointClickCare, strong communication and interpersonal skills, excellent clinical judgment and problem-solving abilities, passion for patient-centered care and functional outcomes
