Occupational Therapist (OT) | Long Term Care

Role Overview

Responsible for coordinating and delivering occupational therapy services to patients across all age groups, developing individualized treatment programs, and evaluating patient progress.

Key Responsibilities

  • Develop and implement personalized treatment plans based on patient assessments and ongoing evaluations.
  • Provide direct therapy services using various strategies and settings.
  • Coordinate treatment with multidisciplinary team members, including physicians and family.
  • Document clinical evaluations, treatment plans, and progress notes accurately.
  • Communicate effectively regarding patient status and needs.
  • Participate in departmental program development and professional growth activities.
  • Supervise and evaluate clinical internships with students from educational institutions.

Qualifications & Skills

  • Active occupational therapist license in the practice state.
  • Ability to perform essential job functions, including effective communication and visual acuity.
  • Preferred 1–3 years of clinical experience.
  • Commitment to compassionate patient care and teamwork.
  • Adherence to safety standards, laws, and regulations such as CMS, OSHA, and The Joint Commission.
