Bring outpatient-level care directly to individuals in their homes and communities. This flexible PRN, W2 role offers autonomy, professional stability, and the chance to make a meaningful impact in real-world settings.

About the role

  • Serve as the primary Occupational Therapist from evaluation through discharge
  • Approximately 15–20 hours per week across 3 days, with consistent client relationships
  • Focus on functional, real-world rehabilitation in homes and community settings
  • Help clients transition back to meaningful daily life, including:
    • Managing and picking up their own prescriptions
    • Returning to driving
    • Transitioning back to work
    • Participating in yoga, light hiking, and community activities
  • Thrive with autonomy while collaborating with an interdisciplinary team of OTs, PTs, SLPs, LCSWs, and other specialists
  • In-person, 1:1 therapy with indirect work (documentation & communication) completed from home
  • Drive time paid at your clinical rate plus mileage reimbursement
  • No weekend shifts

Responsibilities

  • Perform evaluations and develop treatment plans
  • Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences
  • Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
  • Communicate patient’s needs and progress to the treatment team, physician, person receiving services, and family members
  • Collaborate with other disciplines to ensure coordination of care and enhance patient outcomes

Requirements

  • Minimum of a bachelor’s degree in Occupational Therapy from an accredited program
  • Current, unrestricted license as an Occupational Therapist in California
  • Current CPR Certification
  • A minimum of one year’s work experience as an Occupational Therapist
  • Demonstrates knowledge of rehabilitation techniques related to complex neurological diagnoses (preferred)
  • Ability to lift 50 pounds; moving, lifting, or transferring patients may involve lifting up to 100 pounds following safety procedures
  • Fine motor skills, visual acuity, and ability to walk/stand for extended periods
  • Additional physical requirements: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently
  • Effective and professional communication in verbal and written interactions
  • A health screen or examination may be required prior to assignment and periodically thereafter

Pay

USD $60.00 - $70.00 per hour
