Pay
$64–$92 per visit/hour
Schedule
Flexible scheduling with full-time, part-time, and contract opportunities. W-2 and 1099 options available.
Responsibilities
- Evaluate pediatric patients
- Develop treatment plans
- Deliver skilled occupational therapy
- Document services
- Educate caregivers
- Participate in care planning
- Optional COTA supervision

Requirements
- Current Arizona Occupational Therapy license
- CPR certification
- AHCCCS ID
- Article 9 certification
- Valid driver’s license
- Dependable transportation
- Excellent organizational skills
