Occupational Therapist / OT

About the role

Flagstar Rehab is seeking Occupational Therapists for the Rehab Department in state-of-the-art skilled nursing facilities throughout NYC and Surrounding Areas. OTs work with a diverse, multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve physicality and cognition, positively impacting patients' ability to function in daily lives. Flagstar Rehab offers direct employment or independent contractor options, with free assistance setting up a corporation.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for COTAs to follow
• Supervise COTAs daily
• Provide quality therapy services to individuals in an SNF setting requiring care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnoses
• Monitor and document patient progress using innovative EMR software

Requirements

• Current NYS OT License and Registration
• EMR Competence
• State Required Medical Documentations for work in a Skilled Nursing Facility (Physical, TB Test, MMRV Vaccine Record or Titers, Covid Vaccine Documentation)
• Valid Proof of Identity and Work Authorization
• Malpractice/Liability Insurance (if applicable)

Pay

$46-$48 Hourly (W-2), $57-$60 Hourly (1099)
