Flagstar Rehab is Seeking Occupational Therapists (OT) for Skilled Nursing Facilities! Flagstar Rehab puts Occupational Therapists in the driver’s seat when it comes to managing their careers and work-life balance. Requirements

• Current NYS OT License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for COTAs to follow under your supervision.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.

Pay

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Schedule

• Full-Time, Part-Time, Per Diem, Weekends, Days, Evenings, and Coverage Shifts Available
