About the role

Clinton Manor Living Center is seeking an Occupational Therapist to support residents with physical, cognitive, or psychosocial impairments in achieving their highest level of independence in daily activities. This PRN position involves evaluating and treating residents, developing individualized treatment plans, and collaborating with an interdisciplinary team in a skilled nursing facility setting.

Responsibilities

  • Conduct comprehensive evaluations of residents' physical, cognitive, and functional abilities using standardized assessments and clinical observation.
  • Develop and implement individualized treatment plans addressing activities of daily living (ADLs), instrumental ADLs, cognition, and upper-extremity function.
  • Administer therapeutic interventions including therapeutic activities, adaptive equipment training, sensory integration, and cognitive-perceptual retraining.
  • Recommend and train residents in the use of adaptive equipment and home modifications to maximize safety and independence.
  • Monitor resident progress and modify treatment plans based on clinical response and changing needs.
  • Educate residents, families, and caregivers on strategies to support functional independence and prevent decline.
  • Collaborate with the interdisciplinary team, including nursing, physical therapy, and speech therapy, to coordinate resident care.
  • Maintain accurate, timely documentation in compliance with Medicare, state, and facility requirements.
  • Participate in team meetings, care conferences, and continuing education/in-service programs.

Education, Certification & licensure Requirements

  • Master's or Doctoral degree in Occupational Therapy from an ACOTE-accredited program.
  • Successful completion of the National Board for Certification in Occupational Therapy (NBCOT) certification examination.
  • Current, unrestricted state license as an Occupational Therapist (OTR).
  • Multistate practice privileges through the OT Licensure Compact are a plus in participating states.

Experience & Preferred Skills

  • Experience in a skilled nursing facility or with geriatric populations preferred.
  • Proficiency with electronic medical record (EMR) documentation systems.
  • Strong clinical reasoning, communication, and problem-solving skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.
