Occupational Therapist (OT)

Responsibilities

• Evaluate and treat residents in accordance with physician orders and established care plans.
• Develop individualized treatment programs focused on maximizing independence and quality of life.
• Complete timely and accurate documentation utilizing Net Health and PointClickCare (PCC).
• Collaborate with therapy, nursing, and interdisciplinary team members to ensure exceptional patient outcomes.
• Maintain compliance with all facility, state, and federal regulations.

Qualifications

• Current Ohio Occupational Therapist license in good standing.
• SNF/LTC experience preferred, but not required.
• Experience with Net Health and PointClickCare (PCC) preferred.
• Strong communication and organizational skills.
• Commitment to providing high-quality, resident-centered care.
