About the role

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model. Our ultimate mission is to dramatically improve patient access to home health care. The OT role is the front line of that mission: You restore function in the home, build safe routines, reduce avoidable decline, and help patients regain independence in ADLs/IADLs. We build technology designed around the way clinicians actually work, so you can spend less time on documentation and coordination, and more time with patients.

What you will do

  • Complete in-home OT evaluations and deliver skilled OT visits.
  • Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
  • Recommend and train on adaptive equipment and home modifications as needed.
  • Coordinate with nursing and the broader care team when condition or needs change.
  • Document promptly and thoroughly and on time.

Requirements

  • Active Texas OT license (in good standing)
  • Strong clinical judgment and comfort working independently in the field
  • Comfort escalating changes in condition to RN/MD and documenting rationale clearly
  • Strong communication and documentation
  • Reliable follow-through on orders, frequencies, and patient goals
  • Current driver's license and comfort with daily travel across assigned territory
  • Able to carry/transport basic therapy equipment as needed
  • Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)

Nice-to-haves

  • Prior home health experience
  • Homecare Homebase (HCHB) experience (or similar home health EMR)
  • Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
  • Strong organization and time management
