About the role
Provides patient evaluation, assessment, and treatment to promote functional recovery, developing and implementing personalized care plans. Documents interventions, reassesses patient progress, and adjusts treatment accordingly. Plans for discharge, including patient and family education, follow-up, and community referrals. Supervises clinical students and support staff, fostering a caring environment that supports healing. Acts as a resource and participates in hospital committees to enhance healthcare delivery and performance improvement efforts.
Responsibilities
- Assess patients using subjective and objective data.
- Develop and implement written care plans.
- Deliver treatment aligned with established goals.
- Document care in medical records.
- Evaluate treatment effectiveness and reassess patient status.
- Provide discharge planning, education, and community resource referrals.
- Contribute to staff and student development.
- Engage in activities to improve healthcare quality.
- Achieve assigned goals.
Qualifications
- Minimum 3 years of clinical experience or advanced training and certification in primary practice area.
- Basic computer literacy skills.
- Licensure as an OT and BLS certification within 14 days of hire.
Preferred Skills
- Certified Lymphedema Therapist (CLT)
- Advanced specialization or certification
