Responsibilities

  • At the direction of the Clinical Manager, screens, evaluates, re-evaluates and treats patients referred by a licensed provider.
  • Coordinates and communicates treatment activities with health care team members and the patient.
  • Works closely with the interdisciplinary team to ensure high-quality patient care.
  • Evaluates the patient's need for occupational therapy and develops/carries out appropriate treatment plan.
  • Instructs family members, COTA's, Home Health Aides, and other caregivers in appropriate occupational therapy techniques.
  • Communicates with the physician and other health care providers on a regular basis regarding the patient's progress.
  • Provides clinical oversight of COTAs in the home.

Requirements

  • Must be a licensed Occupational Therapist in Vermont and New Hampshire or hold a compact license.
  • Graduate of an accredited school in Occupational Therapy.

Preferred Qualifications

  • One year of experience as an OT in a home health care setting is preferred.
  • Unencumbered OT license in VT and NH or compact.
  • Driver’s license, minimum requirements on car insurance, and a clean driving record.
