Responsibilities

• Screen, evaluate, re-evaluate, and treat patients referred by a licensed provider
• Coordinate and communicate treatment activities with health care team members and the patient
• Work closely with the interdisciplinary team to ensure high-quality patient care
• Evaluate the patient's need for occupational therapy and develop/carry out an appropriate treatment plan
• Instruct family members, COTAs, Home Health Aides, and other caregivers in appropriate occupational therapy techniques
• Communicate with the physician and other health care providers on a regular basis regarding the patient's progress
• Provide clinical oversight of COTAs in the home

Requirements

• Licensed Occupational Therapist in Vermont and New Hampshire or hold a compact license
• Graduate of an accredited school in Occupational Therapy

Preferred Qualifications

• One year of experience as an OT in a home health care setting
• Unencumbered OT license in VT and NH or compact
• Valid driver’s license with minimum car insurance requirements and a clean driving record
