Responsibilities
- Screen, evaluate, re-evaluate, and treat patients referred by a licensed provider
- Coordinate and communicate treatment activities with health care team members and the patient
- Work closely with the interdisciplinary team to ensure high-quality patient care
- Evaluate the patient's need for occupational therapy and develop/carry out an appropriate treatment plan
- Instruct family members, COTAs, Home Health Aides, and other caregivers in appropriate occupational therapy techniques
- Communicate with the physician and other health care providers on a regular basis regarding the patient's progress
- Provide clinical oversight of COTAs in the home
Requirements
- Licensed Occupational Therapist in Vermont and New Hampshire or hold a compact license
- Graduate of an accredited school in Occupational Therapy
Preferred Qualifications
- One year of experience as an OT in a home health care setting
- Unencumbered OT license in VT and NH or compact
- Valid driver’s license with minimum car insurance requirements and a clean driving record
