About the role

The Home Care Occupational Therapist is responsible for the delivery of patient care through the process of assessment, development, implementation, education and revision of treatment programs as necessary for provision of age-specific direct care/services to patients.

Responsibilities

  • Assess patients' needs and develop individualized treatment plans
  • Implement and modify treatment plans based on patient progress and feedback
  • Provide direct care services such as therapeutic exercises, adaptive techniques, and environmental modifications
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care
  • Educate patients and families on self-management strategies and home safety
  • Document patient care activities and progress in accordance with regulatory requirements

Requirements

  • Graduate of an accredited occupational therapist program; Bachelors or Master's degree required.
  • Licensure in the State of Michigan as an Occupational Therapist required.
  • Requires valid Michigan driver's license and dependable vehicle insured as required by the State of Michigan.
  • Home Care experience preferred.

Qualifications

  • Knowledge of occupational therapy principles and practices
  • Ability to communicate effectively with patients, families, and interdisciplinary team members
  • Strong organizational and time management skills
  • Physical ability to perform the duties of the position

Skills

  • Excellent interpersonal and communication skills
  • Proficiency in computer software for documentation and record-keeping
  • Ability to work independently and as part of a team
