Occupational Therapist - Home Health

Role Overview As a Home Health Occupational Therapist, you will evaluate and develop personalized rehabilitation programs for patients in their homes, focusing on improving strength, mobility, and independence. Key Responsibilities Assess patient skills and interpret diagnostic tests to inform care plans. Collaborate with physicians and healthcare teams to develop and revise treatment plans. Deliver therapeutic treatments, instruct patients, and recommend adaptive equipment. Supervise and train therapy assistants and aides. Document patient progress, interventions, and care plans accurately. Participate in care coordination, discharge planning, and community reintegration activities. Design and implement retraining programs and coordinate with interdisciplinary team members. Qualifications & Skills Current, unrestricted OT license and CPR certification. Valid driver’s license with reliable transportation. Preferred: minimum six months of OT experience and home health background. Additional Requirements Comply with TB screening and driver safety policies. Ability to work independently in patients’ homes and communicate effectively.
