Occupational Therapist - Home Health

About the role

Deliver personalized rehabilitation care in patients' homes to improve strength, mobility, and independence. Assess and develop therapeutic programs based on diagnostic tests and patient needs. Collaborate with physicians and the healthcare team to create and modify Care Plans.

Responsibilities

• Provide treatment, education, and adaptive equipment recommendations to enhance patient function.
• Supervise and evaluate therapy staff and aides, and train family members on care techniques.
• Document patient progress, interventions, and discharge summaries accurately.
• Participate in care coordination, discharge planning, and community reintegration activities.

Requirements

• Current unrestricted Occupational Therapist license.
• CPR certification.
• Valid driver’s license and reliable transportation.

Qualifications

Preferred experience includes six months of occupational therapy, especially in home health settings.
