Overview

We seek a licensed Occupational Therapist to provide personalized, independent rehabilitation services in patients' homes and communities.

Responsibilities

• Conduct comprehensive patient assessments to determine needs and develop treatment plans.
• Deliver tailored therapy programs and adjust plans based on patient progress.
• Collaborate within an interdisciplinary care team to ensure optimal patient outcomes.

Qualifications & Skills

• Minimum one year of occupational therapy experience or successful completion of a rehab internship.
• Licensed to practice as an Occupational Therapist in New York State.
• Valid driver’s license required outside Manhattan.

Key Duties

• Practice independently in the community setting.
• Assess and respond to evolving patient needs through plan adjustments.
