Occupational Therapist/Full-Time - Home Health & Hospice

Role Overview

Provides in-home occupational therapy services to patients with neuromuscular, skeletal, and perceptual disorders under physician supervision.

Responsibilities

• Assess patients, develop and implement treatment plans, and document progress.
• Schedule patient visits independently, coordinate with healthcare staff, and manage confidential information.
• Work effectively in various settings, including acute, rehab, outpatient, and home health.

Qualifications & Skills

• Graduate from an accredited Occupational Therapy Program.
• Hold a current state license.
• CPR certification.
• Minimum of one year of relevant experience; two years preferred.
• Physically capable of lifting up to 50 pounds.
• Strong assessment, communication, and problem‑solving skills.
• Good time management.
• Ability to work independently, adapt to change, take initiative, and handle emergencies.
• Valid driver’s license and ability to drive in hazardous conditions.
• Energetic, collaborative, and committed to professional growth.
• Excellent documentation and interpersonal skills.
• Prior experience in acute or home care preferred.
