About the Role
An Occupational Therapist in Home Health assesses and evaluates patients' functional status, activities of daily living, and occupational therapy needs related to medical conditions.
Responsibilities
- Provide therapy within legal scope, following the plan of care, using evidence-based techniques, and collaborating with the healthcare team
- Evaluate patients' functional abilities, document CMS requirements, and consult with physicians on treatment plans
- Monitor, record, and communicate patient responses and condition changes to supervisors and physicians
- Conduct assessments to determine functional levels using diagnostic and prognostic tests
Requirements
- Current Occupational Therapy license in practice state
- CPR certification
- Valid driver’s license, vehicle insurance, and reliable transportation or access to public transit
- Ability to work effectively in diverse home environments regardless of client background
