What You Bring (Requirements)
- Graduate of an occupational therapy curriculum
- Legally authorized, currently licensed, registered or certified if applicable to practice in the state
- A minimum of one year’s experience working with a frail or elderly population required
- Valid driver’s license and means of transportation
- A minimum of one year’s experience working with a frail or elderly population required
What You’ll Be Responsible For
- Conducts an in-person initial comprehensive occupational therapy assessment on all participants upon enrollment and coordinates with the Interdisciplinary Team on a Plan of Care for each participant.
- Conducts an in-person re-assessment minimally every six (6) months or as needed.
- Provides evaluation/treatment on acute, restorative and maintenance levels to participants.
- Functions as a member of the Interdisciplinary Team.
- Maintains regular attendance at, and participates in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
